Indications/Uses
Treatment of upper & lower resp tract, GIT, GUT, skin & soft tissue, biliary tract, bone & pelvic infections; gonorrhea, septicemia, endocarditis, meningitis, peritonitis, dental abscess (as an adjunct to surgical management). H. pylori eradication in PUD. Prophylaxis of endocarditis.
Dosage/Direction for Use
Adult & childn >40 kg 250 mg tid, increase to 500 mg tid for more severe infections. Max: 6 g daily in divided doses. Severe or recurrent purulent infection of the resp tract 3 g bid. Simple acute UTI Two 3-g doses w/ 10-12 hr interval. Dental abscess Two 3 g doses w/ 8 hr interval. Gonorrhea 3 g as a single dose. H. pylori eradication 750-1 g bid in combination w/ PPI & antibiotics for 7 days. Childn <40 kg 125 mg tid. Severe infections May be increased to 250 mg tid. Acute otitis media 750 mg bid for 2 days may be used as alternative treatment. Patients w/ the highest risk of infective endocarditis that require manipulation of the gingival or periapical region of the teeth or perforation of the oral mucosa in adult & childn >40 kg 2 g single oral dose 30-60 min before procedure. Childn <40 kg 50 mg/kg single oral dose 30-60 min before procedure. Moderate renal impairment in adult & childn Max: 500 mg bid. Severe renal impairment Max: 500 mg/day. Childn <40 kg 15 mg/kg bid (max: 500 mg bid). Patients receiving peritoneal dialysis Max: 500 mg/day. Adult receiving hemodialysis 500 mg or 15 mg/kg in childn. 1 additional dose may be administered during dialysis & at end of each dialysis.
Administration
May be taken with or without food: May be taken w/ meals for better absorption & to reduce GI discomfort.
Contraindications
Hypersensitivity to β-lactams (penicillins & cephalosporins).
Special Precautions
Hypersensitivity. Avoid use if infectious mononucleosis is suspected. Overgrowth of non-susceptible organisms in prolonged use. Pseudomembranous colitis. Renal impairment. May increase risk of jaundice in newborns. Phenylketonuria. Sugar intolerance. Pregnancy & lactation.
Adverse Reactions
Drug Interactions
Decreased renal tubular secretion w/ probenecid (resulting to increased & prolonged blood levels). Reduces estrogen reabsorption & efficacy of combined OCs. Increased allergic reactions w/ allopurinol & INR w/ acenocoumarol or warfarin.
